Standard culture testing — the method used in virtually all clinical settings — missed every contaminated case. The biofilm was present and generating an immune response. The test simply could not detect it.

When the implant is removed but the capsule is left behind, the biofilm remains. The immune system continues reacting to the bacterial antigens in the capsule tissue. Symptoms may persist or improve only partially.
