How Do Everyday Environmental Exposures Affect Your Body's Ability to Heal?

When I sit down with a patient before surgery, the conversation extends well beyond what will happen in the operating room. I ask about diet, fluid intake, home environment, and the products they use every day. These questions are not incidental. They help me understand where a patient's physiology is starting from, and how prepared their biology is for what surgery will demand of it.


Recently, I had the opportunity to sit down with Dr. Aly Cohen, a board-certified rheumatologist and integrative medicine physician based in Princeton, New Jersey. Dr. Cohen is the author of Detoxify and the founder of the Smart Human brand and podcast. She has spent more than two decades studying how environmental exposures interact with the human immune system. Our conversation reinforced something I have observed across 30 years of surgical practice: a patient's external environment has a direct relationship with their internal physiology, and that physiology shapes both preparation and recovery.

How Does the Immune System Respond to Everyday Chemical Exposure?


Dr. Cohen opened with a framework I find both accurate and clinically useful. The human immune system is extraordinarily capable. It has been refined over millions of years to identify foreign material, manage inflammation, and restore balance. The challenge facing modern bodies is not that this system is broken. It is that the volume and variety of synthetic compounds the immune system now encounters daily far exceed what it was historically calibrated to manage.


Since World War II, an estimated 100,000 synthetic chemicals have been introduced into everyday life through food packaging, pesticides, personal care products, household cleaners, textiles, and building materials. Of those 100,000 compounds, the vast majority have never been required to undergo formal safety or toxicity testing, particularly in populations with elevated biological sensitivity. These include pregnant women, young children, teenagers navigating significant hormonal changes, and those in menopause.


Many of these compounds are capable of mimicking hormones. Others interfere with the signaling pathways the immune system depends on to regulate its own activity. When the body is continuously managing a high synthetic chemical load, one downstream consequence can be elevated baseline inflammatory activity. For patients preparing for surgery, that has direct relevance. The immune system does not distinguish between the inflammation produced by a foreign chemical compound and the acute inflammation created by a surgical incision. It responds to the total load.



What Is the Best Way to Filter Drinking Water at Home?


Dr. Cohen described water quality as the single most underrecognized contributor to daily health challenges in this country, and I think she is right. The United States public water system serves approximately 85 percent of the population and is regulated under the Safe Drinking Water Act of 1974. That framework formally addresses only 91 chemicals, while an estimated 100,000 or more compounds can potentially enter the water supply through industrial discharge, agricultural runoff, pharmaceutical residue, and aging infrastructure. That includes hormone-disrupting compounds, arsenic, heavy metals, and a broad range of industrial chemicals.


Tap water is not a viable primary water source. The regulatory framework has not kept pace with the chemical environment we now live in.


Dr. Cohen recommends building toward water filtration in stages, starting from what is accessible and most practical:


Carbon block filtration is the entry point. Pitcher filters, refrigerator door dispensers, and shower head attachments all use carbon block technology. They are affordable and remove a meaningful range of contaminants. They are not the most thorough option available, but they are significantly better than unfiltered tap water and a reasonable starting point.


Reverse osmosis (RO) filtration is the most comprehensive residential option currently available. An RO system passes water through membranes with extremely fine pores, catching the vast majority of chemical compounds that conventional filters miss. A countertop unit costs approximately $275. A professionally installed under-sink system typically runs $400 to $450 total, including a plumber's time to split the water line. Over time, RO tends to be less expensive to maintain than carbon block because replacement cartridges are less frequent and less costly.


For travel, Dr. Cohen recommends carrying a stainless steel or glass container and filling it at the carbon block filtered stations available in most major airports, rather than relying on plastic bottled water. When bottled water is the only option, choose varieties filtered by reverse osmosis or distillation over those simply labeled as purified.


At home, I made the switch to an RO system years ago. I tell my patients the same thing I practice: fluid quality is a preparation variable. What you hydrate with affects your biology going into surgery.



How Does Indoor Air Quality Affect Long-Term Health?


Modern construction has made homes significantly more energy-efficient than a generation ago. That efficiency creates a consequence most people do not think about: we have sealed ourselves inside our own air.


The average home contains multiple sources of airborne chemical exposure. Volatile organic compounds (VOCs) are gases released continuously by new carpet, furniture, cabinetry, and paint. Synthetic fragrances in candles, dryer sheets, air fresheners, and cleaning products release airborne compounds into that sealed environment. Dust, which settles on every surface in the home, carries concentrated chemical residue from everything that has entered the space and off-gassed over time. When children and pets make surface-to-mouth contact, that dust becomes a direct exposure route.


Dr. Cohen's recommended approach starts with elimination rather than filtration. Reducing the number of synthetic products entering the home cuts the indoor chemical load at its source. This means simplifying the cleaning product inventory, choosing lower-chemical personal care options, and opening windows when outdoor air quality permits. From there, the next steps are adding plants, maintaining HVAC systems with appropriate filter ratings, and considering HEPA air purifiers as an additional layer when needed.


The sequence she recommends is eliminate first, then ventilate, then filter. It is both practical and cost-effective. It does not require expensive equipment to make meaningful progress, and it addresses the problem where it starts rather than trying to remediate it downstream.


For patients in Texas, I would add that the climate here presents specific challenges. Cedar and mold seasons in Austin are significant because sustained cold temperatures rarely interrupt the pollen cycle the way they do in other regions. For individuals with immune sensitivities, the home environment becomes a particularly important controlled variable during those seasons, because outdoor exposures are difficult to manage and indoor quality is something you can actually control.



What Does USDA Organic Actually Protect You From?


Dr. Cohen made a point during our conversation that I want my patients to understand clearly: in the United States food supply, there is exactly one regulation with meaningful and uniformly enforced protection from synthetic chemical exposure. That regulation is USDA Organic certification.


The USDA Organic standard was developed over nearly a decade in the 1990s and went into effect in 2002. It prohibits synthetic pesticides, synthetic fertilizers, artificial food coloring, artificial preservatives, artificial additives, and genetically modified organisms. It applies to the entire product. If something carries the full USDA Organic seal, every ingredient must meet that standard. Unlike personal care products or cosmetics, where a single organic ingredient can appear on the label while the rest of the formula is synthetic, USDA Organic food certification is comprehensive and uniformly enforced.


Cost and access are real considerations, and Dr. Cohen addresses this directly. Frozen USDA Organic produce retains nutrient density, meets the full organic certification standard, and is now available in most major chain grocery stores across the country, including budget-friendly house brands. Flash freezing locks in nutritional value in a way that long-distance fresh produce often cannot. Fresh produce in a national chain supermarket may have traveled for days or weeks by the time it reaches the shelf, losing significant nutrient value in the process.


For non-organic produce, the Environmental Working Group publishes annual lists identifying which produce carries the highest and lowest pesticide residue. A wash in one part baking soda to three parts water, or one part white vinegar to three parts water, removes a meaningful portion of surface pesticide residue at essentially no cost.


Food quality connects directly to the inflammatory markers and metabolic variables I evaluate as part of preparing a patient for surgery. What a person eats in the weeks before a procedure is a real input into their inflammatory status when they arrive.

What I Have Observed in 30 Years of Surgery


Every patient I see undergoes toxicology evaluation. In more than three decades of practice, I have encountered exactly one patient whose toxicology panel showed nothing in even the moderate range. One person, out of thousands.
